Senior Living Properties Rehab2Home Program motivates residents to enhance their quality of life by improving their functional abilities and successfully transitioning them to a life of independence. At times, your health care provider may determine that you no longer need the amount of care provided in the hospital, but you need more care than you and your loved ones can manage at home.
We understand that most people desire to return home from the hospital, even when you and your physician made plans to make that happen. But sometimes, one’s recovery may be slower than expected and as a result you may, in fact, need to be transferred to a skilled nursing or rehabilitation facility.
Our Skilled Nursing Centers provide both sub-acute rehabilitation and long-term care for our residents.
Residents may stay just for their recovery period, or for an extended length of time.
Each SNF, provides a doctor to supervise your care, along with other highly-trained health care providers who will help you regain your strength.
Residents are evaluated by a team of Licensed Therapists, who thoroughly assess the condition of each person we work with and develop the most appropriate programs for their condition and abilities. Our treatment and exercise ideas are tailored to each individual to make sure the patient enjoys the best results.

Physical Therapy
Our Physical Therapists will focus on bodily strength and movement. We will help our residents recover from a variety of injuries, where extended examinations may be performed. Treatment may combine exercises to build strength and range of motion with pain-relieving techniques. Our strategies help reduce pain in joints and/or muscles. Our therapists work smart to reduce discomfort caused by everything from arthritis to heart disease. Whether receiving short- or long-term care rehabilitation, we’re helping people to stay mobile and safe for a longer period of time.
Occupational Therapy
Our Occupational Therapists will assess your entire range of skills and activities needed to perform daily functions. We teach patients how to dress, cook and get around. We patiently teach the use of assistive devices such as a walker, a wheelchair or a reach-and-grab tool. The goal is for the patient to become more independent. Your occupational therapy treatments will likely be a combination of practicing the skills you will need to go home and address the physical, cognitive, emotional health factors that make these essential tasks difficult. We return our patients to a prior level of functions that were conducive and active, whether it’s bathing, dressing, eating, or participating in physical activities once enjoyed.
Speech Therapy
Our Speech Therapist, also known as Speech-language pathologists (SLPs) provide short-term rehab care and ongoing treatment for long-term care residents. They are trained in areas of oral motor skills such as swallowing, cognitive communication and orientation such as memory and problem solving, and verbal fluency. We can treat patients with any condition that could hinder their ability to communicate. We can address communication, cognition, and dysphagia deficits that may be due to a number of underlying diagnoses. There’s a high concentration on memory and swallowing problems. We can teach individuals how to improve swallowing by working and strengthening their muscles thereby reducing the chances of choking or inhaling food or liquid, a problem seen frequently within skilled nursing facilities. Our Speech Therapists treat patients with dementia, Alzheimer’s, stroke victims, and several other diagnoses that can affect the patient’s cognition, memory, and speech. We also treat patients that have swallowing problems, also known as dysphagia.
